Child Case Management

Description:

This service is designed to provide community support and advocacy to children diagnosed with a severe emotional disorder. It assists the child to succeed in the community and school by identifying, accessing, and advocating for needed services and supports the child with navigating systems such as family, school, or County Child and Family Services. Using a comprehensive case management approach, quality mental health treatment is provided to children and families using the mental health recovery model, a conceptual psychosocial rehabilitation model.


The service provides rehabilitation and environmental support activities that assist clients in reducing the symptoms of psychiatric illness, build resilience and improve their quality of life and integration into the community. These services are relevant to the diversity of the persons served and consistent with their needs. Clinicians assist with the identification, evaluation, and assessment of needs and strengths. Individualized Service Plans are jointly developed and implemented with the individual being served and family or peers as appropriate and permitted.
